Having fired my field rod and load at a javalina from my .50 Hawken reproduction. I can see it.

Seen this qualifying with an M16, 20 years ago. Buddy next to me, left his cleaning rod in too. Shit happens, I guess.

Ya, I've seen this as well. Guy next to me was shooting black powder, left it in after tamping down the ball. Made a really weird sound as it went down range. When he went to reload, I heard him say 'where's my rod'? Too funny..

I suppose if you were moving the rod back and forth and it unscrewed itself due to the brush / patch catching the rifling and you were bullshitting with your friends you might be distracted enough to not notice it wasn't attached when you put the kit away. If it can happen it will happen.

That article is good advice or any AR owner.

But, in all honesty, you are dealing with a lot of new recruits on the quantifying range, and let's be honest....some of them are about as sharp as a spoon. The majority will be completely new to firearms. As in, the first time in their life handling an M4 (or any firearm) was last week.

Isn't the current issued item the OTIS sytem?

I am not sure how that can happen.
I always look down the barrel of my gun after I clean them to see if they are clean.
I have looked down the barrel of every gun I have ever owned after cleaning it.
I look down them before I target practice and I look down them before I go out it the field when hunting.

I did however blow up the cannon on an M42 Duster when it had a double feed while firing it.
The grim reaper missed me that day.
