AR Stripping Primers

Went to the range with an AR I haven’t shot in a bit. Second shot there was a jam. I look in after locking the bolt and a live round is mostly in, but was stopped by a spent primer stuck above the back end of the case preventing it from properly seating. Any ideas? I have had issues with the bolt before getting stuck so it could just be that.
 
A blown primer is usually caused by the ammo. Are these reloads? If not what brand of ammo? Is your barrel marked .223 and the ammo is 5.56 which is a bit higher pressure?

Need a bit more info for a good answer.
 
5.56 marked barrel and I’m pretty sure it was independence 5.56 rounds but I can’t be 100% positive
 
Loose primer pocket
Non staked primer
Too high cartridge pressure
Bad primer
Bad case

99% of the time this is a ammo problem.

Seen it rarely at the range. Thankfully it didn't lodge itself into the gas key.
