Agreed. He dropoed the mag, pulled the action open but didn't check that the chamber was clear. Then oops.

I dry fire my pistol quite a bit at the house. I always double and triple check before I do. Even after that, the very first pull I make is in the safest direction possible...before I get into my practice.

What caused that? Extractor didn’t get good contact or never went into battery?

Nope, He didn't check to see if the chamber was clear so the next round didn't go boom.
I get that was the consequence of not fully checked for a clear chamber. Why didn’t that round clear when he pulled the charging handle?

If the last shot case separated how did the new round get in the chamber?!

Dunno. You asked why the round didn't eject.

When my case separated, the following round jammed the bolt from closing. Friggn PITA to get the stuck case out.

But a case separation won’t fire the next round.

I’ve had case separation before. I agree, 0/5 don’t recommend. It was my own doing. I didn’t fully understand how far I was overworking the brass.

From the video it looks like he only pulled the charging handle partway back.

Released charging handle and rechambered the round he half pulled out.

Pulled the trigger and... boom. N/D.
